So we are asking that each family read this parent agreement to ensure that you know and understand that your involvement, as well as your child’s, is important to their success this year in Faith Formation. * There is a new time, new pick up requirements, and Covid-19 requirements added this year.Our Mission:Our Faith Formation at St. Joseph’s will help your children grow as Disciples of Christ, a process in which you – their parents – have the primary role. St. Joseph’s will do this by providing catechesis for all children on Sundays between September and April (till October for Confirmation). We will give special emphasis to sacramental preparation. (initial) Section A: For a parent who has any child registered with St. Joseph’s Faith Formation: (Everyone)1. I understand that I am my child’s(ren’s) 1st teacher in the faith, therefore I will support and help my child(ren) learn what they are introduced to on Sundays or given at packet pick up. (initial) 2. I understand St. Joseph’s Faith Formation is carried out by a group of volunteers. Therefore my child’s catechist and the Director are not available all the time to assist me. (initial)3. I understand that bringing my child to Sunday Liturgy (Mass) is more important than participating in Faith Formation. (initial)4. I understand it is my responsibility to update the Parish of any address and/or phone number changes. (initial)5. I understand it is my responsibility to stay informed as the year progresses by reading the Sunday bulletin, newsletters and other information my child receives regarding their participation in Faith Formation. (initial)6. I understand attendance is critically important. In the event my child will not attend a session, I will contact either the director or my child’s catechist. (Initial)7. I understand if my child received a scholarship it is a privilege, not a right. If my child does not attend or participate (misses six or more sessions), the scholarship may be given to another child who can use it for the next year. (initial)8. Behavior: Your child is required to be able to behave in the session, if we have any problem with your child, you will be notified and need to take care of your child. If need be, you may need to stay with your child in the classroom, or I understand they may need to be homeschooled by you, the parent. (initial)9. Attendance: Do to Covid-19, I understand that if my child is sick, or there is a chance they have been exposed to someone with Covid-19, I need to notify the director (920-1383) or their Catechist. Initial10. Pick Up: I understand that I am responsible for picking up my child(ren) either at the front of the Education Center (3rd-6th) or the back of the Guadalupe Center (Pre-k-2nd) at the time of 10:35-10:45 a.m. If I am late, my child will be brought to the Parish Hall (social distancing), and a call will be made by either the head Catechist or the director. Remember, we are not a babysitting service! (Initial)11. I understand that keeping everyone safe in the buildings, is important to everyone’s well being. When Faith Formation is being held, the doors will be locked at 9:45 a.m. This will be considered an absence if I do not bring my children on time. Faith Formation begins at 9:30-10:30 a.m.. (initial)12. Payment: I understand that Faith Formation is a total cost of $10.00 per child, your payment is due in full by December 6th 2020, or my child will be dropped from the program. (no scholarships offered this year, this is a ½ price for this program). (initial)Section B: For a Parent who has a child registered in the 1st Eucharist Program (and Parent Session Eucharist 101 program) (1st Eucharist only)1. I understand there are mandatory events in the preparation for 1st Reconciliation/1st Eucharist that my child will need to attend as well. These include: a 1st Reconciliation communal service and a 1st Eucharist Retreat. (initial)2. I understand that if my child misses 6(six) sessions, they will be dropped from the program of preparing for 1st Eucharist, and no refund will be given, (if they are sick or have been exposed to Covid-19) I need to make arrangements to continue their preparation for this sacrament . (initial)3. I understand my child must be baptized before they can receive 1st Reconciliation/1st Eucharist. I must locate and turn in a copy of their baptismal certificate. (initial)4. I understand if my child is over the age of 7 (seven), and have not received the sacrament of baptism, they will need to be enrolled in Faith Formation, but they will be celebrating their sacraments at Easter Vigil next year, no exceptions, please let the director know if your child needs their sacrament of baptism! For those who are post 3rd grade level and need to prepare for their 1st Eucharist, I understand that I, their parent will be required to teach a 12 week session to prepare my child for their 1st Eucharist (beginning in December, this is with the support of the church)the guidelines and forms that need to be signed before my child begins their preparation. I also understand there is an extra fee, this program is called 1st Eucharist 101. (initial)6. Payment: I understand that the payment for 1st Eucharist is $30.00, it is due by December 6th 2020 if it is not paid, I understand my child will be dropped from the program. (initial)Section C: For a parent who has a child registered in Confirmation Preparation: (Confirmation only)1. Parent(s) and Students: I understand it is my responsibility to pay attention to important dates that are mandatory participation, this includes: All Sunday/Online sessions, Attend 1st Sunday Masses, , Any Combined Sessions, The Homeless Meal, Christmas Eve Liturgy, Family/Sponsor Dinner, Saint Paper and Sponsor letter due dates, Lent Giveback, Day of Giving participation, Summer Sessions, and a Mini Retreat (TBA), and other due dates for required items. Session Attendance: I understand that if my child misses 6 sessions of their Confirmation Preparation, my child will be dropped from the program and no refund will be given, no exceptions. (initial)3. Payment: I understand the total payment (which is $40.00) for Confirmation is due by December 5th 2021, if this is not paid, I understand that my child will be dropped from the program. (Initial)4. For Online Students Only: I understand that the whole book must be complete and turned in by our June session. I also understand that my student and parent are required to schedule and attend a Zoom session every month with a catechist or director. I also understand the online session, will have sessions that will need to be attended in the classroom (Homeless Meal, Combined sessions, Summer Sessions and the Mini Retreat) . I understand that if Covid 19 gets worse in Wyoming, Faith Formation will be moved to Packet-Pick up only and Online Sessions for preparation of Sacraments (1st Eucharist and Confirmation). ( Initial)2. I understand that I have the option to enroll in Packet Pick Up, which will be offered on the 1st Saturdays of every month beginning in October 2021 and ends March 2022. If my child is need preparation of Sacraments, I have the option of doing a homeschool/online option for Confirmation and 1st Eucharist (2nd grade only!).
